Position: Case Manager, Independent Living Program
Reports To: Supervisor, Independent Living Program
Pay Rate: $30,225 starting salary based on experience

$2,000 Sign-On Bonus*
*$500 at 90 days, $500 at 180 days and $1,000 at 1 year

POSITION SUMMARY:
The Case Manager for the Independent Living Program works with current and former foster care youth ages 14-23 as they transition to adult life. Worker provides comprehensive assessments and individual guidance, facilitates life skills groups, and helps youth reach goals identified in the individualized case plan. Reliable transportation required for extensive travel through Erie County.

JOB DUTIES:

  • Completes opening paperwork and youth assessment, develops individualized case plans, and facilitates life skills groups and individual life skills sessions.
  • Develops an individual independent living case plan for all youth/young adults in the program, to be updated at least every 6 months
  • Places strong emphasis on coordination and facilitation of life skills groups.
  • Provides case management for youth/young adults ages 14-23.
  • Assists with referrals to other community resources and provides advocacy for youth/young adults to ensure they receive the proper services. Provides support that will enable them to advocate on their own behalf
  • Conducts visits to youth/young adults' home or placement, and goes into the community with them regularly.
  • Provides reliable transportation to appointments, job interviews, school visits, community functions, and other relevant activities.
  • Documents daily service activity to maintain proper case records
  • Provides Erie County Office of Children and Youth and Erie County Juvenile Probation Office with quarterly progress reports
  • Provides youth/young adults with guidance, support, and preparation for independence with appropriate assertiveness, professionalism, and positive rapport.
  • Demonstrates coordination skills while working with the family, staff, and other agency personnel.
  • Acts as a liaison for the referral sources to assist in the referral process, including obtaining signatures for permission to participate in the program from either the biological parent or the OCY caseworker
  • Learns and understands community organizations and their purposes
  • Participates in individual, team and overall agency supervisory and staff development activities
  • Professionally represents the program and agency in meetings, public forums and court appearances, as required.
  • Performs other duties as assigned.

EDUCATION AND EXPERIENCE:

  • High school diploma, GED, associates degree or bachelor's degree required.
  • Minimum of 2 years experience working with families, children or teens (experience volunteering, or working in support role within a community setting is highly valued). 
  • Must have a valid driver's license and access to reliable transportation.
  • A combination of education and experience equal to a minimum of 4 years preferred.
