Family Based Mental Health provides intensive in-home therapy, case management and crisis intervention to help parents with children who have mental health issues or are at risk of hospitalization due to mental health symptoms. Co-therapists work with the family to help children remain in their homes, schools, and communities by preventing hospitalization and out-of-home placement.
Family Based Mental Health is a voluntary 8-month program that generally includes 3-5 visits with the family each week. Counselors are on-call 24/7.
Family Based Mental Health is available in Erie, Crawford, Mercer and Venango Counties.
Family Based Mental Health is available to adolescents ages birth to 21 years old who have a mental health diagnosis and are at risk of hospitalization, out-of-home placement or returning to the home from hospitalization or placement.
